I am conducting some research surrounding the out-of-pocket expense that you incurred when originally applying for a permit for a person with a disability to take me with a crossbow. This does not include costs associated with loss of wages, transportation and/or "inconvenience", etc. Rather, I am attempting to compile (and then average) the amount of money you had to pay in order to have the (1) physical therapist examination and (2) having a licensed physician verify the PT's testing. Please note also what amount, if any, your medical insurance paid as part of the total process.

Finally, if after having the examination/test, you did not meet the criteria , please let me know if that costs was out-of-pocket as well and the amount.

It's been a while but the first time I got turn down was 35 dollars for my doctor visit. I waited for my insurance to get my approved but they kept telling me they had codes wrong and that it was still being checked on so I shelled out 120.00 to get checked out the first time. never got it back from my butthead insurance either:rant:

The second time was through VA and after all the hoopla they told me they didn't do it anymore after making me wait almost eight months. no charge but a lot of wasted time

The third time I got better insurance and the whole thing cost me 12 bucks total between doc and the pt.;) sweet for me:D

hate to have to go and pay a doc and pt out of my pocket now:help:

If I remember right I have about $85.00 involved in getting mine. The $10.00 copay for my doctor and another $75.00 to the Rehab place who did the testing.

I spoke with my neighbor and he got his permit in Summer of '06, he is 72 years old and not a computer person. Total $250

Dr. -just had to pay $25 co-pay as was during regular visit. His Dr. said that it was a joke that he even had to see the PT as if he could , would sign off on the permit... At the time had handicapped license plate ,bad knees, shoulder and wrist from years of cement work.

PT- First one said would not do it ????? Second one said had to do 3 visits at $75 each.... So did that and got the permit.

He took a doe in 06 & 07....

I got mine in '03. Since both my doctor and therapist were familiar with my condition, all I had to do was bring in the form and they signed it, no charge.
However, I believe I was the exception. I know two other hunters who should have permits but have been denied. One had an animal rights wacko for a therapist who refused to sign for anyone, no matter what their condition.
